Consent to the processing of personal data in order to provide publishing services

1. I hereby consent to the processing of my personal data in the following:

1.1. last name, first name, gender, identity document number, identity document validity period, date of birth, telephone number, e-mail address

1.2. other information - other information provided by the author

2. In accordance with Art. 9 of the Federal Law of July 27, 2006 N 152-FZ “On Personal Data”, I give the Yur-VAK Publishing House conscious consent to the processing of my personal data specified in subparagraphs 1.1 and 1.2 of paragraph 1 of this consent (including collection, recording, systematization, accumulation, storage, clarification (update, change), extraction, placement, use, transfer (distribution, provision, access), depersonalization, blocking, deletion, destruction) of personal data for the purpose of fulfilling publishing service contracts.

3. Yur-VAK Publishing House has the right to carry out automated processing of personal data specified in subparagraphs 1.1 and 1.2 of paragraph 1 of this consent, their processing without the use of automation tools, including with transmission through communication channels (including cross-border) , the protection of which is ensured through the implementation of appropriate legal, organizational and technical measures provided for by the legislation on the protection of personal data.

4. In order to achieve the goals specified in paragraph 2 of this consent, I agree that the Yur-VAK Publishing House has the right to entrust the processing of my personal data specified in subparagraphs 1.1 and 1.2 of paragraph 1 to companies that are part of the same group of persons as the Yur-VAK Publishing House -VAK', authorized representatives of the Yur-VAK Publishing House and third parties - partners of the Yur-VAK Publishing House on the basis of an instruction or an agreement concluded with this person (s), or by adopting an appropriate act (hereinafter referred to as the instruction of the operator). I am informed that the responsibility for the actions of the person processing my personal data on behalf of the operator lies with the Yur-VAK Publishing House as the operator that issued the corresponding order.

5. I hereby confirm that I consent to the processing of my personal data specified in subparagraphs 1.1 and 1.2 of paragraph 1 of this consent by the Yur-VAK Publishing House from the date of granting this consent until the date of revoking of consent in written form, without prejudice to the provisions of paragraph 7 below

6. I hereby confirm that I have been informed of the possibility of revoking this consent by personal appeal or by sending a written request (including in the form of an electronic document signed with a qualified electronic signature in accordance with Federal Law No. 63-FZ of April 6, 2011 'On Electronic Signature"), addressed to the Yur-VAK Publishing House.

7. I am informed that the Yur-VAK Publishing House has the right, after receiving the revocation of this consent, as well as after the expiration of this consent (3 years from the date of conclusion of the publishing services agreement), to continue processing my personal data if there are grounds provided for by applicable law.
